You must complete this form if you have a conference scheduled in a Domestic
Relations Section and you are going to claim that you have unusual needs and
unusual fixed expenses that may warrant deviation or adjustment from the
support amount determined under the state guidelines.

The income portion of this form must be completed by all parties scheduled to
attend a support conference. The expense portion of this form must be completed
by all parties when the combined monthly net income of the parties is $30,000
or less and:
- The party is claiming unusual needs and expenses that may warrant deviation from the support guidelines pursuant to Rule 1910.16-5, or
- The party seeks an apportionment of expenses pursuant to Rule 1910.16-5.
At the conference you must provide receipts or other verification of expenses
claimed on this statement.

In Pennsylvania, when a child is born to a woman who is unmarried, there is no legal relationship between the father and the child. A father of a child born to an unmarried woman is not the father for legal purposes unless both parents have signed an Acknowledgment of Paternity form, or a DRS or court has entered an order that establishes the legal father of the child.
