SSN Disclosure Notice (IN-006) 13 K The Social Security Disclosure form is provided to you when applying for Title IV-D support, to inform you of how your SSN will be used in providing service. Read this form before contacting your Domestic Relations Section.
Application for Child or Spousal Support (IV-D Application)

270 K Use this form if you do not already have a court order for support for one or more of your children. Filing of this form will start the court process to get you support payments. Complete this form and then contact your Domestic Relations Section to see if you must deliver the form in person.
Complaint for Support (IN-005) 949 K Use this form if you would like to have a court order established for the support of your child(ren). The Complaint for Support is a required, legal filing to begin a support case. Complete this form and then contact your Domestic Relations Section to see if you must deliver the form in person.
Intake Questionnaire (IN-002) 663 K This or similar information is needed by the Domestic Relations Section to start the process of getting a support order for your child(ren). Complete this form and take it to the Domestic Relations Section when you go in for your first visit.
Parent Locate Questionnaire (LO-001) 306 K Use this form if you would like to provide the Domestic Relations Section with information that may be useful in locating the non-custodial or absent parent. Complete each line of the form with the requested information. Mail or deliver the form to the Domestic Relations Section administering the non-custodial parent's case.
Defendant Contact Information Request (LO-502) 299 K Use this form if you would like to provide the Domestic Relations Section with information on a non-custodial or absent parent of a child support case. Complete each line of the form with the requested information. Mail or deliver the form to the Domestic Relations Section administering the non-custodial parent's case.
Income Statement & Guidelines Expense Statement (IN-008) 353 K You must complete this form if you have a conference scheduled in a Domestic Relations Section and you are going to claim that you have unusual needs and unusual fixed expenses that may warrant deviation or adjustment from the support amount determined under the state guidelines. The income portion of this form must be completed by all parties scheduled to attend a support conference. The expense portion of this form must be completed by all parties when the combined monthly net income of the parties is $30,000 or less and:
  1. The party is claiming unusual needs and expenses that may warrant deviation from the support guidelines pursuant to Rule 1910.16-5, or
  2. The party seeks an apportionment of expenses pursuant to Rule 1910.16-5.
At the conference you must provide receipts or other verification of expenses claimed on this statement.
Expense Statement for Cases Pursuant to Rule 1910.16-3.1 and Rule 1920.31 (IN-009) 603 K You must complete this form if you have a conference scheduled in the Domestic Relations Section and the combined monthly net income of the parties involved exceeds $30,000 This form must be completed when the combined monthly net income of the parties is more than $30,000.
Request an Acknowledgement of Paternity Form N/A Use this form if you would like to establish paternity for your child. In Pennsylvania, when a child is born to a woman who is unmarried, there is no legal relationship between the father and the child. A father of a child born to an unmarried woman is not the father for legal purposes unless both parents have signed an Acknowledgment of Paternity form, or a DRS or court has entered an order that establishes the legal father of the child. Complete the website form to request an Acknowledgement of Paternity (AOP) Form. The AOP form will be sent to you. It will look like this sample AOP Form.
Complaint for Support – Defendant as Petitioner (IN-011) 11 K Use this form if you are a Non-Custodial Parent (NCP) that would like to have a court order established for the support of your child(ren). The Complaint for Support is a required, legal filing to begin a support case. Complete this form and then contact your Domestic Relations Section to see if you must deliver the form in person.
Request for Verification of Insurance Information Form (CM-010) 113 K Use this form if you would like to provide the Domestic Relations Section with updated health insurance information for yourself and/or your dependents. Complete each line of the form with the requested information. Mail or deliver the form to the Domestic Relations Section administering your case.
Job Search Employment Form (EN-020) 51 K Use this form if you have been ordered to complete Job Search Employment Forms in order to find gainful employment. Complete each line of the form with the requested information. Mail or deliver the form to the Domestic Relations Section administering your case.
