The following forms can be printed and submitted to your county DRS. Read the descriptions to determine which forms are appropriate for your needs.

Application for Child or Spousal Support (IV-D Application)
(IN-001)*		 Complete this form to request a new support case.
Complaint for Support (IN-005)* Complete this form to request establishment of a support order. The Complaint for Support is a required, legal filing to begin a support case.
Intake Information Questionnaire/Data Sheet (IN-002)* Complete this form to provide the Domestic Relations Section with information for yourself, your children, and the other party to the support action.
Income and Expense Statement (IN-008) Complete this form to document income and expenses of either party in a support case.
Petition for Recovery of Overpayment in Terminated Case (IN-010)* Complete this form to request recovery of an overpayment in any amount from a closed case. The form must be filed within one year of the termination of the charging order.
Complaint for Support – Defendant as Petitioner (IN-011)* Complete this form if you are a noncustodial parent (NCP) that would like to request establishment of a court order for the support of your child(ren). The Complaint for Support is a required, legal filing to begin a support case.
Request for Verification of Insurance Information (CM-010) Complete this form to provide the Domestic Relations Section with updated health insurance information for yourself and/or your dependents.
Praecipe for Entry of Appearance(Attorney Submitting Entry of Appearance) (CM-200)* Complete this form to notify the Domestic Relations Section of your intention to represent your client at conferences and hearings.
Praecipe for Entry of Appearance – Self-Represented Party (CM-201) Complete this form to notify the Domestic Relations Section of your intention to represent yourself at conferences and hearings.
Withdrawal of Complaint(CM-202)* Complete this form to indicate that your complaint for support can be withdrawn.
Job Search Employment Form (EN-020) Complete this form if you have been ordered to find gainful employment.
Request an Acknowledgement of Paternity form Submit this request if you would like to have an Acknowledgement of Paternity form sent to you. Acknowledgement of Paternity forms are used to establish paternity of a child.
Parent Locate Questionnaire (LO-001) Complete this form to provide the Domestic Relations Section with information on the noncustodial parent for your support case.
Defendant Contact Information Request (LO-502) Complete this form to provide the Domestic Relations Section with information on the noncustodial parent for your child support case.
Petition for Modification/Termination/Reinstatement of an Existing Support Order (OM-501)* Complete this form to request modification, termination, or reinstatement of an existing support order.
Petition for Recovery of Support Overpayment in Active Case (OM-513)* Complete this form to request that the charging order be reduced by 20 percent or an amount sufficient to recoup the overpayment by the time the charging order is terminated.
Summary of Medical and/or Dental Bills (EN-024) Complete this form to report unreimbursed medical and dental expenses to your Domestic Relations Section.
Summary of Additional Expenses (EN-124) Complete this form to report unreimbursed additional expenses to your Domestic Relations Section.
